Looking for a Home Support Worker to provide personal care and home support for an active person with ongoing medical conditions related to chronic illness and previous transplant surgery. The Home Support Worker will track all medication taken, ensure refills of prescription medication as needed, monitor and report overall health, behavioural/personal changes affected by prescription modifications, schedule activities and appointments, maintain records, write reports, and inform related medical personnel of any potential issues or changes in health that are recognized. In addition, the Home Support Worker will prepare meals according to strict dietary needs, and perform additional duties in maintaining a clean, accessible, and organized home. The contract period is from April 1, 2018 to June 1, 2020 and wage is $16.25 per hour for 30-40 hours per week. The Home Support Worker is required to be living in London initially, however must be willing to relocate to a different city and/or different province depending where the person being cared for decides to relocate this summer (to be determined), In addition, the Home Support Worker must be willing to travel as needed to medical appointments, for visits outside the home, and for social or other activities where assistance is needed. hhswcanada@gmail.com
